Practice Closure Announcement
Closure Effective: June 1st, 2025

We wish to inform our patients and community that New Providence Healthcare Associates, Inc. will be closing its outpatient medical practice effective June 1, 2025. This decision was made with thoughtful consideration, and we are committed to supporting our patients during this transition.
Final Appointment Date:
Patient appointments will be accepted only through the month of May 2025. We strongly encourage all patients to initiate services with a new healthcare provider to ensure continuity of care. Our staff is available to assist with referrals and support the transition process.
Medical Records Requests
Records Custodian:
Patient medical records will be securely retained in compliance with Maryland state regulations for the minimum required retention period, beginning from the date of practice closure.

Our administrative office remains open and available to assist patients. You may request a copy of your medical records or have them transferred to another healthcare provider by contacting our Records Custodian.
